Band Shirt and Hoodie Order Forms
The
band shirts are a short-sleeved gray polo shirt with an M embroidered on the left chest and the student’s name and instrument embroidered on the right chest. Band parents may also order a shirt with “Band Parent” embroidered on the right chest.
The
‘hoodies’ are a navy blue hooded sweatshirt with a pouch pocket in
the front. The front of the hoodie says “Meridianville Band” in large maroon and white
letters. One sleeve will contain the
name of the instrument the student plays; the other sleeve will have the name
of the student. Band parents may order a
hoodie, also.

Your
child is required to have one
of these band shirts to wear at the football games, concerts, etc. If your child does not have a shirt, please
fill out this order form and return it to Mr. Weinberg along with a payment of
$20. (2X and 3X sizes are an additional
$2.) Please make checks payable to MMS Band and write your child’s name on the bottom
left corner of the check. Band parents may also order a
shirt. They will be embroidered with “Band Parent” instead of name and
instrument.
